As a new patient, we will need some paperwork from you.  To make it as easy as possible for you we have put the paperwork on our site so you can download it and complete it at home.  You may mail, fax, email or drop it off in person prior to your appointment.  

All you have to do is scroll down through the list of forms as they relate to the conditions listed and choose the forms that apply to you; print and complete them.  If you have any questions about which forms to use, please don’t hesitate to contact us.

ALL NEUROLOGICAL / FIBRO / DIZZINESS / CHRONIC PAIN / METABOLIC / THYROID Patients:

All paperwork listed in this section MUST be back to the doctor AT LEAST 48 hours prior to your appointment or it will have to be rescheduled.  This is so the doctor can have time to review your case and make the most use out of the initial appointment time.  If you have any questions, please feel free to contact our office as soon as possible.  If this is for a Metabolic or Thyroid problem, you do not have to fill out page "2" and "3" of the New Patient Paperwork if it does not apply.
